Your Initial Evaluation will be scheduled for 2 - 2.5 hours.  The Initial Evaluation will consist of the following 3 components:

    1.    A comprehensive needs assessment to determine functioning strengths,
            current concerns and/or symptoms, and a review of your medical history.

    2.    A Qik CPT, which is a computerized visual continuous performance test 
            developed for assessing attention to detail and impulse control.  The 
            EEG Expert Report on Qik CPT is specifically intended for use by 
            neurofeedback clinicians.

    3.    Your initial EEG Neurofeedback session. 

All results will be discussed and explained to you.  After careful interpretation of the above results, we will develop a Comprehensive Neurotherapy Program specifically designed to your needs. 

Options and expectations will also be presented as part of your Comprehensive Neurotherapy Program.  We will further discuss with you payment options based on treatment needs/frequency.

Initial Evaluations are billed at a flat rate of $215.00.  Payment is expected prior to the first visit.
